Glasgow Coma Scale (GCS)
Assess level of consciousness across three domains: eye opening, verbal response, and motor response. Score ranges from 3 (deep coma) to 15 (fully alert).
Eye Opening
Best observed response
Verbal Response
Best verbal response
Motor Response
Best motor response
⚠️ GCS is a clinical assessment tool. Always perform assessment in person and document in the medical record. Scores should be interpreted alongside full neurological examination.
GCS Classification
13–15 — Mild TBI / Alert
Minor neurological impairment. Close monitoring and reassessment.
9–12 — Moderate TBI
Significant neurological impairment. CT imaging, ICU consideration.
3–8 — Severe TBI / Coma
GCS ≤ 8 = coma. Consider intubation for airway protection. Urgent neurosurgery consult.
Always document as E + V + M (e.g., E3V4M5 = GCS 12) to ensure clarity in handoffs.
