GCS Classification

13–15 — Mild TBI / Alert

Minor neurological impairment. Close monitoring and reassessment.

9–12 — Moderate TBI

Significant neurological impairment. CT imaging, ICU consideration.

3–8 — Severe TBI / Coma

GCS ≤ 8 = coma. Consider intubation for airway protection. Urgent neurosurgery consult.


Always document as E + V + M (e.g., E3V4M5 = GCS 12) to ensure clarity in handoffs.
